For those of you who have already self incorporated and wish to be paid through your corporation, there are insurance requirements that must be met before any work is performed. You must provide a Certificate of Insurance evidencing the following coverage prior to the commencement of work, and must notify Synesis Inc in writing at least thirty (30) days prior to renewal, cancellation of or any change in coverage during the placement period.
Note - Some customers explicitly prohibit this type of arrangement.
Workers' Compensation insurance as prescribed by the law of the state(s) in which the work is to be performed. This policy must include a waiver of subrogation in favor of Synesis Inc and Customer. This policy must also include Employer's Liability insurance with limits of at least $1 million per occurrence.
General Liability insurance including Blanket Contractual Liability covering the placement Agreement.
Comprehensive Automobile Liability insurance with limits of at least $1 million combined single limit for bodily injury and property damage for each occurrence covering all owned, hired and non-owned vehicles.
Professional Liability (Errors & Omissions) insurance with limits of at least $1 million for each occurrence.
